I hereby give my consent to OOO ‘SANGUAN’ located at the address: 105118, Moscow, in. ter. d. Municipal district Perovo, Enthusiastov sh., 34, room. 23/3 for automated, as well as without the use of means of automation processing of my personal data, namely - the performance of actions provided for in para. 3 Article 3 of the Federal Law of 27.07.2006 No. 152 FZ ‘On Personal Data’ (any action (operation) or set of actions (operations) performed with the use of automation or without the use of such means with personal data, including collection, recording, systematisation, accumulation, storage, clarification (update, change), extraction, use, transfer (distribution, provision, access), depersonalisation, blocking, deletion, destruction of personal data contained in this application, for the purpose of proper implementation of the following measures

A list of my personal data, for the processing of which I give my consent:

surname, first name, patronymic;
telephone number (home, mobile).

By this application I also give my consent to make direct contacts with me on the mentioned data by means of means of communication to inform me about services, goods and promotions in accordance with Art. 15 of the Federal Law of 27.07.2006 No. 152 FZ ‘On Personal Data’.

This consent shall be valid indefinitely.

This consent may be withdrawn by me at any time by sending the relevant notice by registered mail to ‘SANGUAN’ LLC (105118, Moscow, Municipal District Perovo, Enthusiastov, 34, room. 23/3).
